Japanese Can anyone here do a small tranlation from Japanese to English for me?

    If anybody could just tell me what the few lines in the 'notes' part (or any part really!) of the attached sheet means I would be very, very, very grateful.

    I too will try to translate it for you. The image does look kinda blurry, and some of the words are all scribbled, but i will try my best. I will reply once more if get anything.
    (I am not guaranteeing anything) but if I have the time, i will get one of my friends to help me on this. School is getting preety busy for me also, since we have F-Cat coming up, so i will try my best.
