I beat MGS2 on extreme mode over the summer. Here's the key to beating extreme:

Beat MGS2 on all difficulty levels, starting with very easy, and get every single dog tag. That way you have INFINITE AMMO and STEALTH in the beginning of extreme mod, which make extreme much easier, more fun, and less frusterating ^_^

The bosses SUCK though . . . took me forever just to beat the very last one . . . but gamefaqs.com has some good strategies. To defeat Olga, you have to lay down on the ground and shoot from between the two boxes. If she throws a grenade, get up, run away, come back, and repeat.

The Harrier fight was also near impossible until I found out the correct strategy for it and could pull it off.

Man, I just had the funnest Metal Gear Substance fight of them all. Okay, so it's pretty hard, but it's really fun.

There's this on special VR Mission that Raiden has to do. First it's you against one guard. Then it's you against six guards, two with shields. Then it's you against 6 Arsenal Tengu and 8 guards. Then, finally...it's you against this BIG BIG Gurlukovich Soldier! LOL. It's great, it even has the Godzilla spines going down it's back, complete with eye beams and death/freeze rays! Oh, not to mention the roar, and the BGM...ah, man, it was excellent. I'm not going to give away how to beat it though, but it's gonna keep you busy for a while...considering it's endurance style!

Anyway, the game is chock full of extras. I haven't gotten all of them yet, but I'm still gonna write about the ones I got ;). Anyway, you beat the game once, and you get two special menu selections: Casting Theatre, and Boss Fight.

Casting Theatre is the greatest thing EVER. What it is is basically you changing the skin of important characters to whomever in the game. There's only eight demos, and voiceovers don't change, but it's hilarious. For example, the final Solidus/Raiden showdown, you can have Rose and a Middle Aged Office Lady going at it like Ninja. Or, have the MGS1 Snake (complete with lacking eyes!) going against MGS2 Snake! HEHEHEHE.

Boss Fight is what it sounds like. You go through all the bosses in the game endurance style. Word of advice, if you can, just use Stingers on Vamp. It works. And if you have time, shoot out the lights with the USP, it prevents that Shadow-hold thing he does.

Boss Fight/Survival, same diff. It's pretty much you as Snake or Raiden going against every boss in the game. You have the option of fighting Olga with Raiden, or fight Solidus with Snake. Actually, the fight with Solidus is easier with Snakey-poo. I've found that Solidus has trouble blocking Snake's punches. Wierd...
